Supported Living Services Waiver (SLS)

The Supported Living Services Waiver (SLS), for adults 18+, provides necessary services and supports for individuals with adults with Intellectual or Developmental Disabilities (I/DD) so they can remain in their homes and communities.

Each adult on this waiver is assigned a Supports Intensity Scale level (SIS) that determines the level of support and funding the individual will receive. Working with your Community Center Board (CCB) the adult will be assessed in the following areas to determine which support level they qualify for:

– Individual Basic Support needs

– Medical Support needs

– Behavioral Support needs

Based on the assessment conducted, your adult son/daughter will be assigned a support level which grants them a specific amount of funding for services to meet their on-going needs.

Below is a list of services we support that your SLS waiver funding will cover! You as a parent, guardian, grandparent, sibling, family friend, etc. can be hired on with Colorado Family Caregivers to provide these services and be paid for doing so!

Provided ​​on ​​a ​​short-term ​​basis, ​​because ​​of ​​the ​​absence ​​or ​​need ​​for ​​relief of ​​those ​​persons ​​who ​normally ​​provide care​​ for​​ the ​​participant.
 
  • ​Respite ​​may ​​be​​ provided ​​in ​​the ​​participant’s ​​home, ​​​​the​​ caregivers home, or in the community.​​​​
  • Respite is paid on an hourly basis with a cap of 10 hours within any 24 hour period.
  • Respite can only be used to assist working parents when their child is 12 years or older.
The respite caregiver can be used to assist with children 11 and younger, if the parent is assisting the client with other waiver services at the same time.
 
Pay Rate: $21 per hour
 
Homemaker services are provided in the client’s home. There are two types of homemaker services: 
a. Basic homemaker services include cleaning, laundry, basic household care or maintenance only in the areas where the client frequents. 
b. Enhanced homemaker services includes basic homemaker services with the addition of either teaching the individual or performing extraordinary cleaning 
• These services shall include direct training to the client in performing basic household tasks including cleaning, laundry, household care, meal prep and cooking
• Incidental basic homemaker service may be provided in combination with enhanced homemaker services
• Extraordinary cleaning are tasks beyond routine sweeping, mopping, laundry or cleaning and require additional cleaning/sanitizing
 
Pay Rate: $25 per hour for Enhanced homemaker
                 : $16 per hour for Basic homamker
*Applicable only if client is under the SLS Waiver
 
A ​​range ​​of​​ assistance ​​to ​​enable​​ participants​​ to ​​accomplish ​​tasks ​​that​​ they​​ would ​​normally​​ do ​​for ​​themselves if ​​they did ​​not​​ have​​ a ​​developmental​​ disability
 
• Hygiene, ​​bathing, eating, ​​dressing,​​ grooming
• ​​Bowel​​, ​​bladder​​, and​​ menstrual ​​care​​
• Medication and health monitoring​
​• Exercising​​​
• Money​​ management
• Meal planning
• Grocery shopping
• Assistance with appointments outside the home
     • Medical appointments
     • Banking
     • Household errands
     • Clothing shopping
 
Protective oversight including stand by assistance with any personal care task.
 
Pay Rate: $21 per hour
* Applicable only if client is under the SLS Waiver
 
Service provided to participants to promote self-advocacy through methods such as instructing, providing experiences, modeling and advising.
 
  • Transportation arrangements
  • Safety and safety awareness
  • Finding community resources
  • Medical advocacy
  • Continuous learning
  • Civic duties such as voting​
Pay Rate: $37 per hour

Once you have selected the individual you would like to serve as your caregiver, call us! We will answer any questions you have and once you are ready – we will onboard your caregiver, coordinate with your case manager, and make the transition as easy and quick!
